Brigette’s Technology Consulting and Research Firm (known as the "Firm": Contractor), own and operated by Brenda L. Nelson-Porter, agrees to contract the Firm's Technology Consultation Services to (Contractee).

Contractor agrees to perform the the following services:

• Coach Maximum of Six (6) Staff Members Per Day on How to Use the Specified Technology

• Critique Existing Website and/or Intranet and Present Findings by Email Correspondence within Two (2) Days After Onsite Session

• Make Recommendations to Executive and/or Management Team by Email Correspondence on Means that May Be Warranted to Increase Productivity and/or Clients or Members with Ten (10) Days After Onsite Session

• Maintain Confidentiality

• Advertise Organization's Program Once Via the Firm's Email Distribution List

Contractee agrees to the following terms and to perform the following services:

• Notify Contractor 20 Days Prior to the Start of Session by Email or U.S. Mail

• Make All Necessary Arrangements:

• Ensure All Technologies, such as Internet Connections, are Accessible and Functional at the Designated Location

• Technologies Not Ready for Use will Result in Contractee Paying the Full Fee if Service Cannot Be Performed by Contractor

• For Future Services within the Contract Period and at the Same Cost, Submit Supplemental Form with Payment

• Grant Staff and Clients Permission to Voluntarily Answer Online Research Questionnaires Listed on the Contractor's Home Page: www.brigettes.com

• Pay Contractor in Full at the End of the On-Site Visit Payable to Dr. Brenda Nelson-Porter

• Will Not Hold the Contractor of any Liabilities of any Virus or Computer Failures or Misconducts of Staff or Clients, and so forth

Both parties have agreed upon Two Thousand U.S. Dollars ($2,000.00) as payment for service per hour and a minimum of two (2) hours per on-site visit per day with a maximum of three (3) consecutive days per site visit. Both parties have also agreed that the Contractee will pay the cost of all travel, hotel, and lunch and will make all travel and hotel arrangements for out-of-state visits.

Signatures indicate that is a legal document and service contract. This agreement is effective for one (1) year from the signature date and when both parties sign. Only changes by an agreed Amendment will be accepted to alter this contract.
